Registration Form

    I hereby enroll myself (my child) in Shostakovich School of Music, Art and Dance and agree to pay full tuition and fees. I hereby agree that I will not hold the above-mentioned school or any member of the faculty or employees liable for injuries sustained on or off its premises, or illness of any kind contracted by my child (or me) while a student of Shostakovich School of Music, Art and Dance.

    POLICY STATEMENT

    REGISTRATION INFORMATION

    All students entering our school must go through an audition or trial class to be placed in classes at the appropriate level. Students must submit a signed, updated registration application and policy statement every year before the start of academic-year and summer classes. Students must also pay a separate registration fee of $50 with each application (allocated for administrative use throughout academic school year for that student). Students who are continuously enrolled for 12 months without breaks or interruptions will not be required to pay more than one registration fee per year.

    TUITION POLICIES

    Families of registered students may either: 1) pay upfront for a full year, 2) pay tuition in installments per each paid session: Session 1 Sept-Nov, Session 2 Dec-Feb, Session 3 March-June. All tuition must be paid in advance. Payments are due by the first class of every month or installment plan. Full-year tuition and installment session plans are significantly discounted. Sibling discounts and other discounts apply as well.

    Shostakovich School of Music, Art and Dance accepts payments by cash, check, and Zelle. Please also note that there will be a 5% late fee for any monthly or installment session plan payments made after 5th of the month, 10% late fee after 10th of the month and 15% late fee after the 15th of the month. If tuition is delinquent by two weeks, the student will not be admitted to class until payment is made in full. There is a $35 fee for each returned check. Accounts that are overdue by 60 days or more may be turned over to a collection agency.

    Refunds are subject to the following conditions:

    • Registration fees are not refundable under any circumstances.
    • Full refunds (minus registration fees) must be requested in writing and received by the school’s office PRIOR to the first class.
    • NO REFUNDS WILL BE GIVEN ONCE THE STUDENT’S INSTALLMENT SESSION HAS BEGUN.
    • All tuition charges are calculated on a per-lesson basis.

    There are only 2 make-up classes per installment session plan and one make up class per monthly payment allowed for classes missed for reason of illness, verified by a doctor’s certificate. However, classes missed due to illness may be made up only within the paid period of the absence. There are no refunds or pro-rating for classes missed due to personal reasons, vacations, schedule conflicts, or terminations during the month.Excessive absences from weekly classes may require a student to be moved to a lower level class. We thank you in advance for your understanding and cooperation.

    • In exceptional cases, students may be eligible for a partial refund if they must withdraw from classes due to prolonged illness (more than one month) or severe injury, as verified by a doctor’s note.
    • PLEASE NOTE THAT ALL CLASSES ARE SUBJECT TO CHANGE WITHOUT NOTICE; SUCH CIRCUMSTANCES DO NOT ENTITLE STUDENTS TO A REFUND.

     

    POLICY STATEMENT

    ATTENDANCE

    Consistent attendance is critical to consistent progress. Students/parents must report all absences to the school prior to class time. Lessons missed due to a teacher’s absence will be made up later. Students can only attend 2 “makeup classes” per installment session plan or 1 “makeup class” per month/4 week session. If severe weather closes public schools in the area, dance classes will be canceled for the day. There is NO REFUND.

     

    COMMUNICATION

    Important school notices and schedule information, including changes and holiday closings, are posted on the school bulletin boards,on the Announcements page of www.shostakovichschool.com, as well as on Facebook. It is the responsibility of each student/parent to check postings before and after class.

    NONDISCRIMINATION POLICY

    Shostakovich School of Music, Art and Dance admits students of any race, color, sexual orientation, national and ethnic origin to all the rights, privileges, programs and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, sexual orientation, national and ethnic origin in administering its admissions policies, scholarship programs or placement decisions.

    ADDITIONAL RULES AND REGULATIONS

    • Every student in the school is expected to behave in a disciplined and responsible manner and to read, sign, and follow the written Code of Conduct provided at registration. reserves the right to suspend or dismiss any student whose behavior or attendance are not in agreement with school policies.
    • To protect the privacy of all our students, parents may not photograph children in classrooms, hallways or elsewhere at our school
      without permission from the office.
    • School administration reserves the right to choose students for specialized programs, concerts, performances and
      competitions. Parents should understand that NOT all students will be selected for such events.
    • School administration also reserves the right to change a student’s class to a more suitable class if the teacher deems
      a transfer is in the student’s best interest.
    • Since Shostakovich School of Music, Art and Dance is a nonprofit organization, students participating in special events will be required to pay additional fees relating to costuming, staging, music, etc. Separate fees will also apply for competitions.
    • Shostakovich School of Music, Art and Dance is not responsible for lost or stolen property/belongings.
    • All children under 12-years-of-age must be picked up by a parent/guardian who will be waiting for the child 10 minutes
      before the official end of class.

    CODE OF CONDUCT

    We ask each person in our community, but especially students:

    • to conduct themselves with honesty and integrity
    • to speak courteously to each other at all times and without abusive, profane or disrespectful language
    • to abide by the policies of our school as regards attendance, dress requirements, and nondiscrimination
    • to treat school facilities and property, as well as the property of others, responsibly
    • to follow all posted safety rules and regulations
    • to avoid any disruptive or distracting behaviors in the classroom or anywhere on school grounds

    By signing below, I hereby attest that I have read and discussed the Code of Conduct with my child and that we agree to uphold and support the principles and expectations expressed herein.

    PHOTO, VIDEO, AUDIO AND INTERVIEW RELEASE

    Unless otherwise noted, as the parent/legal guardian of 

    my signature below grants permission for my child or ward to be photographed, videotaped and/or interviewed by Shostakovich School of Music, Art and Dance or any of its authorized agents, and consent for the publication, broadcast, or other use of the student’s images and/or words for the purposes of promoting our school. In addition, I release Shostakovich School of Music, Art and Dance, or any parties acting on their behalf and with their approval, from liability for such uses of my child’s images and/or words. All such videos, images, words and any other likenesses shall constitute property of our school, without compensation to me.

    School’s Rules and Policies Handbook

    • A non-refundable registration fee of $50 will be collected each September or at registration time.
    • All payments must be made in advance of each three-month period, or at the start of each month if paying monthky. Family discounts are given for those with more than one child at the school.
    • All payments must be made at the front desk.
    • Cancellations are not accepted beyond 72 hours after registration.
    • There is no eating, drinking, or gum chewing allowed on the schools’ premises.
    • All parents must drop off and pick up their children in the front lobby area of the school only.
    • Students are expected to arrive on time and be ready to participate for their classes in the appropriate attire.
    • Make up classes have to be scheduled with the administration and must be taken during their paid session. Your child is entitled two make ups per paid 3-month session, or one make up per paid month.
    • If you cannot attend your make up session, you must notify the school of the cancellation 24 hours prior to the class or that make up will be forfeited. No make ups for a make-up class, and no roll over to the next session!
    • Attendance and Punctuality: Students are expected to attend classes regularly and to be ON TIME. Repeated absences and/or tardiness disrupt the class and hinder progress. A student may be asked to sit out and observe if she/he is too late. Please call the studio if you anticipate tardiness. The school must be notified if a student will be absent from class and prior permission must be granted to leave class early. There is no reduction or refund for lessons missed. Make-up classes are encouraged and must be made up within the same pay period in the same level ballet class or lower.
    • Cancellations due to weather: In case of severe weather, we will send an EMAIL to notify of cancellation or will post on our Facebook page. We are not automatically cancelled if the public schools are closed, as weather conditions often change later in the day when classes take place. We do not add classes or make up classes cancelled due to weather.
    • Class Conduct: Students are expected to be attentive, courteous, and kind. There is no talking between students during class and they need to raise their hand before speaking. All students are expected to use the bathroom before class. We strive for an atmosphere of discipline, while motivating each student to work to their fullest potential and still having fun and feeling encouraged.
    • Attendance & Commitment to Dance Group Classes: Consistent attendance is very important to the growth of each individual dancer, as well as the group. No dancer wants to let the rest of their group down, so we have, in place, attendance rules. No more than 2 absences from a rehearsal for any reason per any 3-month session during academic school year. If there are more than 2 absences, the dancer may be removed from the upcoming performance or the schools’ final show. If your child is injured, we ask that they continue to come to rehearsals, but may sit out. Extra rehearsals may be called prior to a performance if needed. Attendance at extra practices is also mandatory, regular absence rules apply. It is also important to point out that a student may not miss the last rehearsal before a performance (unless specifically excused by the instructor). Placement back into the dance will not occur until there is another rehearsal. The school will allow a private lesson that specifically works on material for the rehearsal you missed to allow for ONE make up of a rehearsal.
    • Recitals/Performances/Costumes Fees: A recital fee (non-refundable per student) is due in September or at initial registration. The recital fee helps cover the cost of performances (for instrumentalists) and costs of production, rehearsals and insurance of our group class performers and dancers for the shows/performances. This fee is a one-time yearly fee and is in efforts to keep any and all future tickets/performance costs negligible. With this fee, you will have participation and receive tickets to your children’s performances and recitals at no extra cost. The group class/dance families will be required to purchase costumes and/or accessories for any upcoming performances (for each dance/number that your child is participating in) with the costumes’ initial deposit always due in September (or at registration), at the initial payment at the beginning of the academic year. Costumes will not be ordered unless deposits are submitted.
    • Tuition Fees: Families of registered students may either: 1) pay upfront for a full year, 2) pay tuition in installments per each paid session: Session 1 Sept-Nov, Session 2 Dec-Feb, Session 3 March-June, 3) pay monthly.       The school accepts payments by cash, check, and Zelle (dianaelya1@gmail.com). Please also note that there will be a 5% late fee for any monthly or installment session plan payments made after 5th of the month, 10% late fee after 10th of the month and 15% late fee after the 15th of the month. If tuition is delinquent by two weeks, the student will not be admitted to class until payment is made in full. There is a $35 fee for each returned check. Accounts that are overdue by 60 days or more may be turned over to a collection agency. Also, accounts with balances will not be able to perform at events without refund of the fees. All cancellations must be made 24 HRS prior to the lessons. All lessons cancelled less than 24 hours prior will be paid for. Thank you for your help in keeping your payments up to date!

    1. Assumption of Risk: I understand that participating in music activities involves certain risks, including but not limited to physical injury. I voluntarily assume all such risks on behalf of my child.

    2. Release and Waiver: I release Shostakovich School of Music, Art and Dance, its instructors, staff, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in music activities at the music school.

    3. Medical Emergencies: In the event of a medical emergency, I authorize Shostakovich School of Music, Art and Dance to seek necessary medical treatment for my child. I understand that the music school will make reasonable efforts to contact me as soon as possible.

    4. Responsibility for Personal Property: I understand that Shostakovich School of Music, Art and Dance is not responsible for any loss, theft, or damage to personal property brought by my child to the music school premises.

    5. Media Release: I grant Shostakovich School of Music, Art and Dance permission to use photographs, videos, or other media taken during music activities for promotional purposes, including but not limited to website, social media, or brochures.

    6. We are parents of children of the Modern Dance class. We allow our children to do (practice/perform) acrobatic elements and incorporate these moves into dance choreography.
    In case of an accident, we will not accuse and/or charge the school for negligence. (Optional)

    I have read this waiver of responsibility form and fully understand its contents. I acknowledge that I am signing it voluntarily and agree that it shall be binding upon me, my child, and our legal representatives.
